Moved GetOnlinePakKey to the scriptpubkey manager; the other blinding
stuff remains in the CWallet. At some point after the rebase we should
consider whether there's a better separation to be had between the PAK
related stuff now that we have wallet boxes.
This one took a little bit of work. Basically everywhere that Andy's new code
didn't compile, I looked for a similarly shaped line in the pre-PR diff between
Bitcoin and Elements and adjusted it in the same way. Was pretty typical; all
CAmounts become CAmountMaps, etc.
I then spent about 7 hours chasing down a coin selection failure in the blocksign
functional tests, which ultimately turned out to be a Core issue. I added a
stopgap fix (commented, search for "stopgap" in wallet.cpp), and opened the issue

There was also a bug where we were not estimating change output sizes correctly
in case we had massive blinded outputs, which I had to fix to get the functional
tests to pass.
asp: the point of this PR was to completely decouple libbitcoin_server from
bitcoin_tool; it removes $(LIBBITCOIN_SERVER) from the `elements_wallet_LDADD`.
I put this back in because our additional code related to pegin claiming makes
heavy use of methods from libbitcoin_server.
If we want to morally accept this PR then we need to do a nontrivial refactoring.

Currently we explicitly disable BnB when there are preset inputs selected or when the subtract fee from outputs option is enabled. This PR enables BnB for both cases.
Kind of an alternative to #17246 (implements the subtract fee from outputs part of it) and borrows a test from there too.

Allows SelectCoinsMinConf and SelectCoins be able to switch between
using BnB or Knapsack for choosing coins.
Has SelectCoinsMinConf do the preprocessing necessary to support either
BnB or Knapsack. This includes calculating the filtering the effective
values for each input.
Uses BnB in CreateTransaction to find an exact match for the output.
If BnB fails, it will fallback to the Knapsack solver.
